You actually get a short bad end (The game calls it ‘Game Clear’ instead of ‘Game Over’) before being directed back to the decision in which you chose to break the controller.

It is Monaka herself; while Servant is seemingly unaware of her exact motives, he is in fact cooperating with her plan to bring Komaru to the Soldiers of Hope. Only because he thinks the end result will bring hope, however.
A rough translation of the lyrics to Progressive - 漸進 -. I say it is rough because, as it turns out, translating a song by ear without an isolated vocal track is harder than I remember it being. All the same, the general meaning should still be intact. Please enjoy!
